ADAP form for my child for Driver's License (eADAP)

* FOR PARENTS OF TEENAGERS GETTING THEIR DRIVER'S LICENSE:  

If your child took HIGH SCHOOL HEALTH in middle school they will need to get the eADAP certificate to get their Driver's License.
